1. This document constitutes Amendment No. 5 to Con-
tract ntered into as of 6 January 1964 by and
between Hair - omic, Inc. and the Government represented
by the Contracting Officer executing this Amendment.
2. In consideration for shipments made under this
contract FOB2 Contractor's Plant, the contract price is
increased in the amount of $1,064.48 to a new total price
of $178,398.48.
3. All other terms, conditions, and requirements of
this contract, as amended, remain unchanged and in effect.
4. Please indicate your acceptance of this Amendment
No. 5 by the execution of all three copies hereto. The
original and one copy are to be returned to the undersigned;
the remaining copy may be retained for your files.
