Adoption of Standard Form 95 (Claim for Damage and Injury)
1. This office wishes to recommend that this Agency adopt and
utilize Standard Form 95 (Claim for Damage and Injury). Mr. John
Finn, Claims Division of the Department of Justice, advises us that
this is almost universally used in other Government agencies and is
of great value in tort claims against the Government.
2. The form requires the claimant to specify specifically the
dollar amount of his claim, and once having done so, is precluded
from increasing the claim. In addition, the form will have certain
advantages in minimizing the correspondence that is necessary every
time a claim is filed against this Agency.
