By COB, 29 July, it will be necessary to have completed Form 2630 Employee Confidential Statement of Employment and Fiancial Interests. 2. As in the past Office Directors' and Deputy Office Directors' statements must be submitted to the DDA. Others completed within the office must be held on file in the office concerned. Reference provides appropriate guidelines for those who must complete Form 2630. 3. By COB, 29 July, each office will submit to the DDA a list of all individuals who have completed Form 2630. It.is requested that instead of submitting a memorandum in lieu of Form 2630 if there has been no change in the information provided on the form that the actual form by completed with the .indication that there are no changes. It implements Executive Order 11222, dated May 8, 1965, and Civil Service Commission regulations as revised June 9, 1967. (1) GENERAL (a) Information supplied in accordazice wlth this subparagraph will be reviewed only by the officials designated herein, who are responslble for ensuring it is not discussed with or made available to others. (b) Notwithstanding the filing of the annual report required by this sub- . paragraph, each employee will at all times avoid .acquiring a financial interest that results, or taking an actlon that could result, iri a viola- ' tion of the conflicts-of-interest provisions- of Section 208, Title i8, United States Code. These provisions state generally that no Govern- - ment employee may participate personally or substantially, as a Gov- ernment employee, in a matter which affects his financial interest, that of his family, or that of an organization in which he is an officer, director, or employee. (2) PERSONNEL REQUIRED TO FILr., Agency personziel will file employnent and financial interest statements if they are. in any of the categories specified in subparagraph (a), (b), or (c) immediately below: (a) Personnel paid at a Ievel of the Federal Executive Salary Scheduia - (b) Personnel classified at GS-13 or above, or at a comparable pay level, in positions determined by the appropriate Deprzty Director, Operatirp ~_ Official, or Head oP Independent Office to be positions whosf: incum- bents are responsible for making a. Government decislon or taking a Government action regarding (1) contracting or procurement; (2) administering or mouitorin ; grants or subsidies; (3) regulating. or auditizzg private or other non-Federal enterprise; c. (4) P:ROCEDIIRES AI~TD RESPONSIBILITIES (a) Personnel required to file statements are to use Form 2630, .Employee Confidential Statement of Employment and Financial Interests. An employee is not required to include any information about his con- . nection with or interest izi a professional society or a. charitable, reli- gious, social, fraternal, recreational, public service, civic, or political organization not conducted as ~. business enterprise. Fo: this purpose, educational and other institutions doing research ar.d development or related work involving. grants of mozzey from or contracts with the Government are considered business enterprises. (b) Statements, including supplementary statements, will be marked "Em- ployee Financial Statement, Eyes Only'" and forwarded to the appro- priate. Operating Official or Head of Independent Office. S~aternents of Operating Officials and their deputies will be forwarded to the Deputiy Director conccrne.d. (c) Ti~.e Deputy Director, Operating Official, or Ileacl of Tndepe.ldent Office will examine each statement and will comment whether in his opinion the duties of the employee involve the organizations, persons, or real property iz1 which the employee has a financial interest or an em- ployment relationship. If the comment. is affirmative, the statement and the comment will be forwarded to the General Counsel. if tizc comment is negative, the statement will be held, in an "Eyes Only" file in the immediate office of t'rze Deputy Director, Operating Official, or Hcad of Independent Office. (d) The General Counsel Zvill review statements foz?wardec to h:rn with ? affirmative comments and cletcz?mine whether a conflict or apparent conf]ict of interest exists, co;isulting with the person submitting the statement and the Deputy Director, Operating O:"ficial, or :lead of Independent Office, as appropriate. If he determines a conflict or ap- pearance of conflict does exist, he will advise th'e appropriate official and return the statement to him for retention.
