One of the basic problems of modem medicine is the prevention and
eradication of the more widespread and serious infectious diseases

surgical methods of treatment have, however, considerably decreased

has not been fully eradicated.
In the control of children's infections, especially poliomyelitis)
little progress has been made thus far. At present, a special
institute
of poliomyelitis has been organized which has prepared an
experimental

enteric encephalitis, brucelloois, and tularemia; new diagnostic
prep-
arationa have been developed for the early diagnosis of diphtheria,
enteric infections, tularaAa, etc. Infectionints and
microbiologists,
hovever, have not improved the control of infectious diseases, and
littl-
vork has been done on developing polyvaccines. More research has
been
done, however, on cardiovascular diseases and cardiovascular
surgery.
Work on the development of now antibiotics has been progresoing
slowly in. the USSR. Fbr example, in the US, 16 institutes are
working
on the development of new antibiotics, but in the USSR there are
only
Wo such institutes.
Considerable success has been achieved in obtaining chemicothera-
poutic preparations. According to the Pharmacological Comittee of
the
lanistry of Health USSR, 72 preparations were issued in 1955.

the production of endocrine and vitsmine preparations is not as

Scientific research in chairs of higher eductional institutions
has declined in the past years, although numerous qualified
personnel
are employed in these chairs. Fbr example, in the 200
scientific research
institutes there are 7063 persons vith the degree of doctor of
sciences,,,
and in the 87 higher educational institutions, and institutes
for the
advanced training of physicians there are approximately 1,700
persons
Vith the degree of doctor of sciences, but these personnel are
not being
fully utiliLed at present.
Insufficient research is being conducted in hospitals and
sanitrxy-.
epidemiological stations, even, in the major hospitals - &-.
funds; are

tary Inspcctorate should be transferred from subordination of

tnry work. It is also necessary to retain the independent
organization
of sanitary-epidemiological stations, which are basic
institutions for
Sanitt-try-epidemioloaical vork and prophylaxis. The role of the
sanita-
tion physician must be raised. A special institute for school
hygicne~
must be organized where specialists, scientists, and physical
culturists
cem properlyeducate school children in maintoining public health.
The problems confronting medical science are of considerable con-
cern. To minimize these, It becomes necessary to reorganize the
direc-.
tion of scientific research. The Ministry of Health USSR and the
AcadeW
of Medical Sciences USSR are now developing measures for
transferring
the planning of medical science to the Academy of Medical
Sciences.
Closer ties must be developed between the USSR and foreign
countrics in:
scientific fields, especially throuSh personal contact and
litera:--e
exchange.
It is now necessary to organize an independent Ministry of
WdLical
Industry which will be responsible for the production of
medicines, med-
ical apparatus, and Instruments necessary for practical public
health
and scientific researOb lutitutions.

The Soviet government must transfer from the Ministry of instru-
ment Building and Automation Equipment, the Ministry of Wat and Dairy
Productep and the 11inistry of Chemical Industry to the ~tinistry of
Health
USSR those institutions which fulfill orders directly and exclusively

The ui_u_`~ method of surgical treatment is pneumotomy. The
time limits for
this operation, In the opinion of the Majority of physicians
and surgeons, is
established to be 6-1,2 weeks nfter the beginning of the
disease, following this
treatment recovery Is noted in 50-75% of cases. However. th~
study of remote
results of conservative treatment of lung aoscesses shows
that this figure is
exaggerated. Expectant tactics in presence of abscesses of
the lungs has no
theoretical basis. In practice such treatment frequently
results in grave com-
plications, such as gangrene. appearance or numerous
abscesses. purulent me-
tustases. empyema, haemorrhage, etc. Often the use of
antibiotics only obscurel
the clinical course of the abscess and conceals its
transformation into the chronit
form. Surgical treatment in late stages still given a high
lethality rate (from 12
to 34% according to different authors). The earlier the
operation is performed,
the lower the mortality. All the above shows that the tactics
of treatment of acutt
lung abscesses should be revised. Pneumotomy should be
pertormed at the ear-
liest possible date. When gangrene sets in incision and
drainage of the necrotic
area of the lung tissue should be performed before the
appearance of a cavity.
The authors consider that the most effective method of
pneumotomy to debride-
ment of the focus with sealed (underwater) drainage. In: the
absence of pleural
adhesions, this operation should be performed In one stage.
The lung to to be
sutured to the Intercostal muscles around the supposed place
of incision of the
abscess. In correct operative techniq%ter with simultaneous
administration of
antibiotics Incision of the pleural cavity In not dangerous.
The authors' conclu-
sions are supported by personal observations. Is)
