Arthrodesis combined with elongation may be Indicated in paralytic
extremities.
Conforty proposed a procedure whereby a 'Z' elongation of the
femur and tibia
through the joint followed by skeletal traction met these two
requirements simul-
taneously. The author proposes 4 other possibilities, in all of

principle is to follow up resection of the articular surfaces of
the knee joint by
distraction of the 2 bones as much as is needed, and to fill up
the gap with bone
graft material. In one case the author keeps the bones distracted
with 2 solid
grafts, and fills up the gap with bone chips and with material
taken from the
patella. Another possibility is to keep the bones distracted by
means of a special
acrylic hollow cylinder with an opening through which the gap may
be filled up with
bone chips. In the third case the gap is kept distracted and
filled up with a few
solid bone grafts interposed between the I bones. In a fourth
modification. the
author fills up the gap with a solid piece of bone taken from the
bone bank or from,
a fresh cadaver.
