Univ., Budapest**~r~.TIUNG. 1958, 6/2 (24-28) Illus. 5 There is an increasing number of complications. Sixty-seven cases with infiltra- tibi ; or abscess formations surgically treated are analysed. The great percentage of complications after penicillin injections is connected with the phenomenon of allergy (proved in 18 cases from 28). It is considered that the choice of the site of the injection is of great importance. After an analysis of the anatomical peculi- arit~es of the gluteal region a method Is given. the aim of which is to be certain of avoiding the gluteps maximus muscle. The operator's hand with a maximally abducted thumb is placed on the hip region, with the thenar eminence at the upper palpable tip of the trochanter major, the thumb touching the anterior spina lliaca and with the index finger on the line of the, trochanter. The spot lying near the head of the second metacarpal bone of the operator is the most suitable for the injection. It must be made in 2 steps: through the skin and subcutis (lot) and through the illotibial tract of the fascLa lat.& (2nd). This method applied to 20.000 instances his not given complications. Sixty-seven cases of surgical complications referred to the authors' hospital were examined in the course of 6 months. The average duration of treatment was 62 days. The pathogenic effect of penicillin allergy in the occurrence of abscesses after injections is discussed. The authors' new technique is described, which consists of avoidance of the gluteus maximus muscle and of accurate determination of the point of injection with referen- I ce to 3 osseous prominences of the hip (anterior iliac spine, gieater trochanter and eminentiacristaeiliacae). The injection In done in 2 stages; Ist the skin and s.c. tissue are penetrated, and with a 2nd movement (to avoid the epifascial layer) the aponeurotic plate of the ileotibial tract Is pierced. With this technique 20.000 in- jections have been given without complications. Finally, restriction of indication of intragluteal injection is proposed. Abstract: [Authors' German summary modified] The authors stress the great advance made in surgery by the use of antibiotics. The mortality Li rate of Wogen infections decreased dramatically. This usefulness of the antibiotics is somewhat reduced by the drawbacks which are present. Therefore, their use has to be considered carefully, especially in surgery. In the hospital, the number of resistant stapbylococcus strains increased from L* to 90 per cent in the last decade and 60 per cent of the personnel became carriers. A year-long discontinuation of the use of penicillin did not improve the picture significantly. The authors advocate a directed application of antibiotics based on determinations of the sensitivity of the causative agent. Further limitations of their prophylactic use is also necessary.
