Tl~e appearance of pulmonary hypertension considerably modifies the symptomatol-

symptoms &-rolv progressively worse, with typical findings at cardiac catlicterizatioll

An operative method for dogs is described. The opening of the atrium and location
of the site are performed by a bloodless technique, but to avoid the risk of air
embolism the blood is readmitted before the actual defect is made. Defects of consi-
derable size which are still open after 6-8 months can be made in this way. These
dogs are suitable for haemodynamic studies.

Experiments were carried out to test a new surgical procedure. In contradistinc-
tion to the surgical methods described so far, the stenosis is not oftIv resected,
but the defect is replaced with a graft. In dogs, the gap was covered with auto-
grafts (skin, muscle) and homografts (fresh and stored aorta) . Afew weeks after
this operation. the pulmonary artery was narrowed and. in spite of the increased
pressure in the right ventricle. this tissue could permanently replace the removed
piece of myocardium. Aneurysm, rupture or stenosis did not develop.

Iramplanted into the wall ol the heart 'Mag. Seb6szet. 195G, 9/1 (50-57) 1 Bus. 12
Ex riments made on dogs in order to clarify the behaviour of different tissues

transplanted into the wall of the heart led to the following results: (1) for the
closure of septal defects a freshly, transplanted piece of norta appears to be the
most satisfactory, then skin; (2) similar results were obtained in the -repair of
artificially produced defects in the conus arteriosus. The elastic wall of the aorta
can replace heart muscle even in circumstances where the pressure in the right
,chamber is greater than normal; (3) for the repair of the left atrium skin, muscle and
aorta are equally suitable. This is of practical value in commissurotomies in which,
-on account of hypoplasia of the auricle, it is difficult to enter the auricle. Here the
trans men have
founslantation permits the operation through the atrium. The experi ts

the defect caused ~by resection was repaired with skin. Healin Yd without

Following the example of Bailey, Swann and others, the present author has also
attempted to devise a technique of retrograde aortic commissurotomy. Just as
mitral commissurotomy is done through the small natural sac of the ati-luln.
aortic commissurotomy is done through a small sac of plastic material, rubber
or pericardium, which is attached to the lateral wall of the ascending aorta. A
new feature is the use of a graft of abdominal skin; the advantages of this meth-
od over earlier techniques are enumerated. Two cases have been successfully
treated in this way. Naef - Lausanne

Clinical and experimental data indicate that in cases of circumscribed narrowing
of the aorta or pulmonary artery atrophy of the elastic fibres develops peripheral
to the stenosis. The decrease of the systolic -diastolic pressure difference is
considered !o be the main cause of this atrophy. It may be found far peripheral to
the narrowing, even at a level free from haemic turbulence. It is inferred that in
the enlargement of the post-stenotic blood vessel divisions a primary role is
played by the decrease of amplitude and only a secondary role by turbulence.
However. besides these 2 factors a role is also ascribed to a hypoplastic develop-
ment of the vessels. Temesvdri - Budapest (IX, 5)

and the shunt volume enlarged in 8 dogs. although the decline in pre-,.iurc was
diminished due to reduction of the pressure in the systemic circulation. No morpholo-

pendiomid, the conditions were just the reverse of those in animali: decrease of the
left-right shunt and decrease of the aortic pressure, without change of the pressure
in the pulmonary artery. It is concluded that in the beginning (re-sults of animal
experiments) an active vascular tone allowing dilatation plays a role, whereas later
(findings in the patient) irreversible increases of resistance in the pulmonary circula-
tion develop as a -result of anatomical changes of the pulmonary ves'sels.

On tile basis of the experience in the surgical treatment of 500 cases of mitral, 28
cas,cs of pulmonary and 6 cases of aortic steno%is the author colnes to tile conclusion
that often tile digital dilatation is not enough. It is often necessary to uw. instrument-
al dilatation and one'should select the valvulotome not only according to the site of
the ostium, but also with due regard to the anatomical quality and structure of the
ostium. Not every ostium can be transected by the use. of a single instrument and the
author believes that his good results are attributable to the right choice of valvillot-
orne. Different kinds of instru ments are described. 11 M. Munich (XVIII, 9)

MaIvular and infundibular stenosis of the pulmonary outflow tract can be treated by
open resection of the pulmonary tissue, Using this method, the cheat was entered
through the left 6th intercostal space. The pericardium was incised. Tile length
of the infundibular stenosis was palpated. An autogenous treeskin graft was
sutured over the proposed area of resection by interrupted sutures, except for one
margin which was left free where the sutures were placed but not tied. The free
edge of the graft was raised by forceps and near its lower margin an Incision into
the right ventricle was made with a Graefe's scalpel. A piece of the heart wall, 2
to 2_1 cm. in length and about 5 mm. in width, corresponding to the infundibular
stenogis, was cut out of the heart wall. The sutures In the free margir of the skin
graft were tied. The stenosis was alleviated not only because some of the obstruct-
Ing tissue was removed but also because the grafted area expanded during systole.
The details of one patient who had a successful opera~tion are reported. After more
than one year the skin graft grew perfectly into the myocardlum and an aneurysm
did not develop. After the obstruction to the pulmonary circulation was overcome
the patient's condition improved. This improvement was confirmed by laboratory.

ABSTRACT: The Mesozoic rocks rest with marked unconformity on a
deformed group of deposits of questionable Paleozoic
age. On going from south to north, the older Mesozoic
horizons are cut out of the section. The Lower
Cretaceous deposits consist everywhere of dark gray
clays with seams of sands and sandstones; but the Upper
Cretaceous contain two principal types of rocks:
limestones on the south and sandstones with layers of
sandy marl on the north. The least thickness of the
Upper Cretaceous rocks is 10 m. on the crest of the

Cherkessk horizon he notes three viell-delineated microfossil
Globorotalia subt)otinae, G. aragonensis, and G. crassaeformis zones.

ABSTRACT: Almost all the Lower Cretaceous rocks are elastics.
Clays are dominant; sandstones and conglomerates are
subordinate. Lower Cretaceous sediments, which are
several thousand meters thick, accumulated during
intense downwarping of a geosynclinal basin. Geo-
synclinal conditions were preserved in the area of
the present southwestern depression of the Caucasus
until Lower Paleogene. In the northern part of the
region, sedimentary accumulation was accompanied by

younger horizons in a northwesterly direction was identified,
formed principally at the expense of Upper Aptian-Lower

these layers bear different names (Belays. Glina, Solonka
