The plain X-ray showed a group of irregular shadows of medium density, excretion pyelography revealed a functioning upper calyx in the affected kidney. Intracapsular left nephrectomy was performed. The lower seg- ments of the kidney removed were shrunken, calcified, and had an uneven surface and yellow dis colou ration.' The tuberculous aetiology was proved by histological examination. The effect of thiopentone in combination with alcohol was found to be higher by 2576 than with thiopentone alone, while the effects of the mixture lasted longer than those of thlopentone alone. The toxicity is higher by 101%. No adverse effects upon respiration were found In either dogs or humans, while in rabbits themixture markedly depressed respiration in the same way as thiobarbiturate alone. Arterial blood oxygen saturation gave satis- factory values. No effects were found upon liver tissue and none with regard to blood coagulatilDn. Leucopenia. was observed after the application of the mixture, but Increase of leucocytes is met with in operations as a result of several mechanisms. Blood alcohol level has an exponential form in the early phase, later it changen into a linear type; none higher than 0.8% was found. Increased diuresis and decreased elimination of Na. K and Cl was observed. The mixture did not affect body temperature. Thiopentone-alcohol mixture may be designated as suitable for artuesthesia even in elderly patients. Adverse properties of thio- pentone are compensated by the effects of alcohol and the efficacy of thiopentone is potentiated by alcohol.
