SUFJECT CIRIS-72 Forms and Instruction Books
1. In accordance with your telephone request today, we are -sending
you herewith the following materials:
(a) 6 packages of 1000 sheets each of Form 2812a (targeted)
(b) 1 pacakge of about 1500 sheets of Form 2812 (fiscal surnmary)
(c) 1 package of about 7-800 sheets of Form 2812b (descriptive)
These numbers are approximate, since the printing plant did not
pack them in the manner we requested. We have plenty more of all,
if you need a resupply.
2. Likewise, herewith are 200 copies of the CIRIS-72 green book on
Guidance and Instructions, per your specific request. Here, too, we can
resupply you if needed.
3. Please be sure to destroy any remaining supply you may have of
the CIRIS forms used last year. It would be quite inconvenient and un-
necessarily time consuming if we should receive submits on the old forms,
inasmuch as our computer procedures are keyed to data codes and sequences
on the 72 forms which differ from 71. Thank you.
