1. The Office of Logistics has been tasked to provide an
input to the Agency's Annual Occupational Safety and Health
Report which is being prepared by the Office of Security for
submission to the Department of Labor. Attached is the referent
memorandum with guidelines and the questionnaire for preparing
the annual report.
2. The OL Safety and Health Committee, in accordance with
subject report. In this regard, I would appreciate the
Committee's completion of the questionnaire, except questions 2,
3, and 40 applying the questions to OL's health and safety
program*
and Is responsible for submitting statements for inclusion in the
Safety and Health, overseas OL's safety and health program
3. Your input is required by 19 February 1982 to soot a
23 February suspense date.
