The corporate name "Mutual Benefit Health & Accident Associa-
tion" has been changed to "Mutual of Omaha Insurance Company."
The organization has become a Mutual Legal Reserve Company.
This will not change the benefits provided in the policies
owned by the members of your organization, nor will it affect
any other provision relating to premium.
If convenient, would you pass along the enclosed notices to
the policyowners shown on the attached billing.

Always striving to expand its protection and service, Mutual of Omaha
reached a new high in 1961. Each business day an average of more than
5,100 benefit checks were delivered to sick and injured policyowners.
The evidence of progress within an organization is most obvious when
changes occur. Actions taken at the Annual Meeting this year included
the adoption of the charter amendments shown on the reverse side. These
progressive changes enable Mutual of Omaha to continue to provide
"Maximum Benefits at Minimum Cost" with the best possible service.

tion of this amendment, the assessment clause in your policy is no longer
in effect. This will not change the benefits provided in your policy, nor
will it affect any other provisions relating to premiums.
