The Government requests a monthly report from of
Cullinet Software, Inc. covering the following issues for this and other
contracts specified. Any charges to be requested by Cullinet whether they be
education units, support units, hourly wage for Cullinet employees or cash
reimbursement for travel and per diem must be reflected in this report.
Requested for the personal services contract (#87*B282900*000) is the
following:
For each cullinet employee charging hours during the month, specify number
of hours, cost for those hours, a summary of their Agency related
activities during the month, their planned tasks for the next month and
"issues" that must be acted on. A summary of year to date hours and cost
shall be included in each monthly report.
Requested for contracts containing education units (1185*101700*000,
s/w , and # 87 _ ed _units ) is the following:
Presently there are three contracts from which units may be expensed. When
one is exhausted, only the ones with remaining credits shall be reported
on.
For each contract specify the number of units spent during the month, the
number allocated for courses not yet attended, and the number remaining in
the contract. Give a breakdown by student .of all units expended and
allocated showing student name, course title, number of units required,
dates, location, etc.

Requested for the application software purchase contract
(# s/w ), is the following:
For each Cullinet employee involved with the loading and maintenance of the
software, specify the hours expended, cost of those hours, a description of
their activities, plans for the next month, and issues requireing action.
For the support units received from this procurement, specify the
expenditure of each unit showing date, employee, summary of Agency related
activities, and planned activities.
The expensing and tracking of education units received from this
procurement is covered in an earlier paragraph.
The above requested items shall be combined into one report and presented to
the Contract Officer's Technical Representative (COTR) no later then the 10th of
each month.
The period of performance is 2/19/87 to 9/31/87.
Should it be determined that the CIA computer environment is no longer
appropriate for the software, the CIA has the right to move it to another
computer.
Payment for the software will be made upon its installation and written
