(Budapest) 1956, 9/4 (381-391) In 1954, Klebsiella pneumoniae was recovered once or repeatedly, on 94 occasi- ons, from the throat of 52 premature and normal infants, 2 children, 2 adults. as well as from the CSF of one infant. When there is an outbreak it may be pre - sent in as many as 50% of the patients as a saprophyte. In such cases, no corre- lation exists between the presence of Klebsiella pneurnoniae in the throat and pneumonia. Many of the infants in the study who had the organism in their throat had no disease whatsoever. In such cases the organism tends to disappear from the throat spontaneously. In those patients with pneumonia due to this organism the onset was acute, the temperature rose steepl- and the disease extended rapid- ly over a whole lobe, several lobes or both lungs. There was leucopenia or the absence of leucocytosis and this-lessens the likelihood of the disease being due to pneumococcus. In such cases one should adminisler both penicillin and strepto- mycin. Occasionally a combination of the tetracyclines and chloramphenicol is more effective. Early diagnosis would be enhanced if blood cultures, typing of Klebslella pneumoniae and the agglutination tests would be used more often. Kagan - Los Angeles, Calif. The infections were all in hospital patients. The infections were severe, moderate and mild. There is a greater frequency of this organism becoming pathogenic or perhaps even becoming more virulent. ((;ppsala) 1958, 47/2 (172-178) This rnethod was used to find the causative agent in 87 children with pnL!un)onia who would not cough. The results of posterior pharyngeal swabs were compared. The causative agent was found in every one of the patients. The organisms grown from the subglottic swabs were more efficient than the tests made from the throat. The cultures as recovered from them were more accurately indicative of the cause of pneumonia. Subsequently, ih-vitro, testing of the recovered orgahisurt was ac- conijOished. In 23 of the 87 instances the original treatment should not have been successful because of resistance as measured. The in-vitro testing is not always of clinical significance. By the time results of such testing reached thectinician, the original treatment had proved successful. This discrepancy occurred less of- ten with the subglottic cultures than with the throat cultures.
