Three personal cases of reversible comatose episodeii are reported. They appear-
ed in cirrhosis with predommantly sclerotic -vascular changes and were caused by
an excessive supply of ammonia to the brain. The term 'Sherlock's po-tal encephal-
opathy' is preferred for this peculiar picture. The patients were fairl) young
per&)ris with hepatotropic noxious influences in their past histories and 8-iffering
from various kinds of cirrhosis. Symptoms were rapidly progressive nornnolence,
tremor, plastic rigor, deltrwm (in one case catatonic state), or periods with
psyclionnotor irritation etc. , passing into coma. without haemorrhages and with an
arornatic smeU issuing from the mouth. These states last for 18 hr. to 12 days and
may recur repeatedly. The) ohould be differentiated from the genuine hepatic coma
of yellow atrophy. They regress spontaneously or following administration of
giutamic acid. Two physioloirical filters are insufficient: the liver. wIL.ch it; circurn-

venttd. and the nervous system, writen is insufficient for enzymatic neutralizalion.

Othet genic factors may rest in the liver (parenchymal damaRe) and in cere
bi-al changes accompanying chronic alcoholism with a IoNv-proteln diet, mallsive
gastrointestinal haemorrhageti, excessive supply of methionine. etc. , all of which
may lead to overcharging of the portal system with ammonium. Great physical
exertion or massive paracenteses may also favour the development of thfis syn-
