1. Headquarters has reviewed the accident report submitted by
letter of transmittal dated 11 January 1957. le.S are in general concurrence
with recommended action.
2. Specifically, Headquarters is taking precautions to negate am future
similar occurrences

a. We are adopting as operational 30? the use of gust alleviator, in
the flour position on cliMbout to WMEAD 5,900 and on, descent from KWMEAD 5,000.
As you may knew, following the loss of #316 in BOCIDER, Headquarters recommended
use of gust alleviators on climbout up to MEAD 5,000. VS assume this has been
adopted as normal procedure throughout the Detachments. We did not, however,
specifically recommnuituse of gust alleviators an descent from KUMEAD 5,000.
This will be clarified as an operational procedure.
b. All FRILITTall will be informed that abnormal oxygen pressure
oxygen systommalfunotion will be absolute and positive cause for immediate abort.
c. All mums will be instructed to descend to cabin MMEAD 1,000
it oxygen supply falls below normal safe pressure.

will be activated by the canopy jettison handle.

a warning device which will indicate to KWOLITILe that 11 has reached maximum
allowable airspeed. This indicator may be some warning sicnal inside the
communicatima eystem rather than an external horn which may not to heard with
helmet and gear on. It may even in some type of vibrator attached to the rudder
pedals which will give an indication. We propose to have WA into this (b)(3)
device and discuss their approach with a-olarmts at KWCABLE.
3. The Director of Operations plans to discuss all aspects of the
accident with during their IDT at Headquarters the last (b)(3)
