III. Findings
2. (U//A ) The OIG found that the system message received by the complainant did not
result because the form r staff member was actually accessing the document in question. The former
staff member did not have permissions to the folder where the document was contained and the file access
audit log data did not show that the former staff member accessed the document in question during the
week the incident occurred. The OIG did find that the former staff member's access on the
had not been removed, requested that account be terminated, and received
confirmation of that termination. The OIG did not find any evidence to support that the former staff
member accessed the document in question following departure from the 01G, and as such the system
message received by the complainant was likely received in error.
